Department of Pediatrics №2

History of department

The department of pediatrics No 2 in Bogomolets National medical university was established on 1, October, 1944. The rector of the institute L.I. Medvid took the decision for establishment of some pediatric departments at the same time: Propedeutics of children’s diseases, faculty pediatrics and hospital pediatrics for qualitative training of children’s doctors.

The first head at department of hospital pediatrics was professor Lev Yosypovych Finkelstein. He managed the department since 1944 until 1948. Lev Yosypovych enriched the department with glorious traditions from the generation of those pediatricians, who graduated from medical faculty at Saint Volodymyr University and launched the formation of Ukrainian pediatric school. Lev Yosypovych Finkelstein became one from founders for establishment of pediatric faculty (1934) and department of children’s diseases in that faculty, which he managed since 1934 until 1941. He organized the special course on training of micropediatricians. At his initiative in Kyiv one of the largest in USSR children’s clinic for 100 beds was established, which became the base for department that he managed.

The first composition of department of hospital pediatrics in KMI in 1944: head at department, professor L.Y. Finkelstein, assistant professor P.M. Nesterovska, assistants B.M. Miasnykova, F.S. Merzon, P.M. Berenstein-Yampolska. In 1946 assistant professor L.Ye. Zeldych and assistant A.A. Weisfeld came to department. The scientific developments at department, devoted to problems on disorders of nutrition, rheumatism, tuberculosis, pneumonia, were actively implemented into practical medical activity.

After death of L.Y. Finkelstein the department was headed by assistant professor L.Ye. Zeldych for a short period of time.

In the same year since 1, September (order at KMI No 504, dd. 01.09.1948) the department of faculty and hospital pediatrics were again united on the base of hospital clinic under supervision of professor V.G. Balaban.

In the next academic year the department of faculty pediatrics moved to the new clinical base and the department of hospital pediatrics was untied with department of Propedeutics of children’s diseases under supervision of professor Olena Mykolayivna Khokhol.

Since 1950 the training of doctors-pediatricians was carried out due to the new curriculum, which anticipated the six years period of training for future children’s doctors.

Olena Mykolayivna Khokhol left the large footprint in the development of pediatrics not only in Kyiv medical institute but in the state in the whole. The efficient methods for treatment of tuberculosis, gastrointestinal diseases at children of the tender age, rheumatism, pneumonia, septico-toxic diseases of infants were developed and implemented into practice under supervision of Olena Mykolayivna, the technology on production of ionite milk was established, the standards for nursing children in the first year of life were developed.

In 50-60-ies the following persons worked at department: professor O.M. Khohol, assistant professors and assistants L.Ye. Zeldych, V.D. Ott, T.S. Berezhanska, T.A. Baraboy, A.A. Weisfeld, O.I. Lazarev professor Liubov Yeremiyivna Zeldych, who preserved and continued the scientific direction of department that had formed during previous years. The results from scientific researches, made by employees at department under supervision of L.Ye. Zeldych, were generalized in monograph “Rheumatism in children”. It described the issues on diagnostics and treatment of rheumatism in children, which had the great practical importance.

The vivid page to history in departments of hospital pediatrics was written by professor Ivan Mykhaylovych Rudnev (10.06.1910-9.07.1970), who became its head in 1965. He was the author of over 80 scientific works, including 1 textbook and 3 monographs and handbooks. His doctoral dissertation “Functional state of vessels at rheumatism in children” made the significant contribution into determination for the main pathogenic mechanisms on development of this serious disease, which rate was extremely high at that time. The scientific works by professor I.M. Rudnev was ahead of their time by many years. The directions in diagnostics and treatment of allergic and cardiac diseases, launched by him and his followers, remain in the center of attention and today’s generation of scientists.

Monograph “Practical cardiology of children’s age”, written together with professors V.M. Sidelnykov and P.S. Moshchych, became the first Ukrainian manual on children’s cardiorheumatology. The works by I.M. Rudnev on application of glucocorticoids to treat autoimmune diseases in childhood, which were widely published in USSR and abroad, remain to be classical until now.

I.M. Rudnev paid the great attention to development of algorithms for differential diagnostics of acquired injuries of the heart at children, development and implementation of new research methods (polycardiography, arterial oscillography, etc.) into practical health care. The scientists at department (L.O. Bezrukov and V.G. Mygal) under supervision of I.M. Rudnev developed the new method for treatment of bronchial asthma and allergosis at children through hyposensitization using bacterial antigens that allowed significantly increasing the efficiency in treatment of those pathological states.

Professor I.M. Rudnev established the whole scientific school. 5 doctoral and 19 candidate’s dissertations were defended under his supervision. His pupils were professors were O.I. Lasytsya, P.S. Moshchych, I.V. Bagdasarova, M.A. Mokhort, L.O. Bezrukov, Ye.I. Volchanskyy, assistant professor V.G. Mygal, Candidate of Medicine G.V. Shafarenko. Professor I.M. Rudnev was the initiator, compiler and first editor of textbook “Children’s diseases” but he could not finish this work because of early death in the pride of his youth and scientific talent. The career of professor I.M. Rudnev was completed by his companion-in-arms and inheritors professors P.M. Gudzenko, V.M. Sidelnykov and P.S. Moshchych, who published this textbook in 1973.

The author’s collective of the first Ukrainian textbook “Pediatric Diseases”, which summarized many years of experience of many generations of pediatricians in the country, was awarded the State Prize of the UkrSSR in Science and Technology in 1979.

The name of corresponding member in National Academy of sciences, Academy of medical sciences of Ukraine and Russian Academy of medical sciences, Honorary scientists and technician, professor Viktor Mykhaylovych Sidelnykov (15.10.1928-31.05.1997) was written into history of national pediatric science with golden letters; he became the head at department in 1971. V.M. Sidelnykov was the founder of children’s cardiorheumatology and allergology; he trained 8 doctors and 37 candidates of science. He wrote over 300 scientific works, 3 textbooks and 12 handbooks and monographs, patented 16 inventions.

At that time, the staff of the Department of Hospital Pediatrics had almost doubled compared to previous years. In the seventies, the following departments worked productively: Assistant Professors T.S. Berezhany, O.S. Kostin, M.A. Makarenko, V.Y. Dashkivska, Y.M. Afanasyev; assistants I.M. Budnik, V.G. Boryakovsky, L.O. Bezrukov, T.V. Pashun, O.I. Lasitsa, V.G. Migal, S.S. Kazak, G.V. Shafarenko, V.Y. Kazachek, M.M. Srebny, L.V. Tereshchenko, I.M. Kryvoruk, N.M. Tsareva.

In 1975, on the initiative of Professor V.M. Sidelnikov, the Kyiv City Children’s Clinical Hospital No.2 (Chief Doctor – Honored Doctor of Ukraine, Associate Professor O.L. Dziuba) was created, which became the clinical base of the department and one of the leading and best medical and preventive institutions of national health care. For the first time in Kiev in hospital were opened Specialized departments of pediatric allergy, cardiology and resuscitation.

At the initiative of Victor Mikhailovich were established pediatric cardio-rheumatology and allergy centers which are working fruitfully today.

The main scientific directions of the department in the 70-80 years were pediatric cardio-rheumatology, allergology and intensive care in pediatrics. His doctoral dissertation Sedelnikov devoted to the problem of diagnosis of congenital heart defects. He was first diagnosed and described the “smallest” defect in the interventricular septum.

Under the leadership of V.M. Sidelnykov first studied the issues of assistance of children with congenital heart defects, the basics of treatment of cardiovascular failure in children with the use of cardiac glycosides, developed a fundamentally new methodology for their use in pediatric practice. For the first time in the country were developed algorithms for the diagnosis and treatment of myocarditis, myocardial dystrophy, allergic Wissler-Fanconi subsepsis, arrhythmias in newborns and older children. Was created the concept of antioxidant protection and metabolic correction of myocardium affected by inflammatory and dystrophic process. Pediatric practice has introduced the latest diagnostic methods – echocardiography, Doppler cardiography and cerebrovascular Doppler cardiography.

His pupils devoted their works to actual problems of pediatric cardiology under the leadership of V.M. Sidelnykov: L.V. Tereshchenko, S.S. Kazak, L.I. Golopikho, A.J. Kuzmenko, A.O. Chascheva, N.V. Balykina, O.P. Volosovets, S.P. Krivopustov, A.A. Pomytkin, A.Y. Lagutin.

Scientific developments in cardiology formed the basis of the fundamental textbook “Cardiology of the Childhood Age” (1986), edited by V.M. Sidelnykov in conjunction with Professors P.S. Moshchyk and D.Y. Kryvcheny.

Many scientific works have been devoted to the problem of diagnosis and treatment of bronchopulmonary diseases, respiratory allergies and bronchial asthma in children. This direction at the department was actively developed under the leadership of V.M. Sidelnykov by such specialists as O.I. Lasitsa, T.V. Pashun, M.M. Srebny, N.M. Tsareva, L.R. Pomitkin, O.L. Dziuba, M.P. Prokhorov, O.M. Rudnev, V.E. Khomenko, O.V. Rybakov and L.P. Arabskaya.

System algorithms for the treatment and prevention of asthmatic bronchitis and bronchial asthma in children have been developed. The clinic has created a microclimate chamber of salt mines with sodium chloride aerosol and negative ions. It was also developed an original “Sidelnyk’s Ointment” for the treatment of atopic dermatitis.

At the department for the first time in the world for the treatment of chronic nonspecific lung diseases in children, a non-steroidal anti-inflammatory agent “Pyrimidant” was applied, which was patented in 8 leading countries of the world (in particular, in the USA, Great Britain, Canada, France, Switzerland).

The results of many years of scientific research in the field of allergology are reflected in three editions of the textbook “Children’s Diseases” (1973, 1975, 1984), in the monographs “Pathological Syndromes in Pediatrics” (1977), “Practical Allergology of Childhood” (1985), “Diathesis in Children ”(1990),“ Pharmacotherapy in Pediatrics ”(1993).

In the 70’s and 80’s, emergency assistance to children became a priority of the scientific and practical direction of the department’s research. On the initiative of Professor V.M. Sidelnykov, the Department of Anesthesiology, Resuscitation and Intensive Medicine was opened in Children’s Clinical Hospital No. 2, which was the second in Kyiv at that time. New principles of post-syndromic treatment of childhood critical conditions have been developed. These scientific developments formed the basis of the manual “Emergency care in pediatrics”, edited by V.M. Sidelnykov in 1976, which was widely recognized in the country and abroad and has sustained three publications.

On the initiative of Professor V.M. Sidelnykov since 1975, issues of perinatology have been widely included in the teaching program of childhood diseases. This direction at the department for many years was led and developed by M.D. Makarenko M.A. In 1989, a separate neonatology course was created at the department, headed by Professor A.S. Lyavnets. Experienced pediatricians and neonatologists were involved in teaching the course: assistants I.O. Loginova, O.F. Cherniy, L.M. Geleskul and S.M. Gnatyuk.

Recognition of his merits was the appointment of V.M. Sidelnykov Chief Pediatrician of the Ministry of Health of the USSR, and then – Chief Pediatric Cardiac Rheumatologist of the Ministry of Health of Ukraine and Deputy of Head of the Society of Pediatric Physicians of Ukraine. For a long time V.M. Sidelnykov was the dean of the pediatric faculty and the vice-rector for scientific and medical work of the Kiev Medical Institute. For his services to the country, V.M. Sidelnykov was repeatedly awarded military and labor orders.

The only professional Ukrainian journal “Pediatrics, Obstetrics and Gynecology” was retained during the most difficult times of economic hardship due to his talent and authority. He was editor-in-chief of this journal more than 10 years.

V.M. Sidelnykov was one of the first medical professors who took part in providing medical assistance to the pediatric population of the areas that had been evacuated from the Chernobyl disaster region.

After the Chernobyl disaster he opened a new line of work in the clinic – the development of problems of environmental pediatrics, which later formed the basis of his activity as a member of the National Commission on Radiation Protection of the Population of Ukraine.

Assistant of the department O.P. Volosovets began a scientific study of the health of children affected by the accident. He proved the influence of radionuclides and other ecotoxic factors of the polluted environment on the development of Acquired Heart Diseases in these children and developed ways for complex pathogenetic therapy of these conditions.

For their scientific and pedagogical achievements, the staff of the Department of Hospital Pediatrics was repeatedly recognized as the best at the Kiev Medical Institute and was awarded of Rectorate. In the early 80’s the department was recognized by the Ministry of Health of the USSR as a basic for teaching children’s diseases for students of the senior courses of pediatric faculties. The scientific developments of V.M. Sidelnykov and his pupils have also been repeatedly honored with VDNH medals in Moscow.

In the early 90’s, the department also established a course in medical genetics.

In 1994, the Department of Hospital Pediatrics was named the Department of Pediatrics No. 2 with courses in medical genetics and neonatology and began teaching pediatrics to medical students at the II Medical Faculty of the University from II to VI years of study.

In 1992, V.M. Sidelnykov was one of the first Ukrainian pediatricians to open a new page of international cooperation of pediatric doctors in our young independent country. He was the scientific adviser of the Ukrainian-American program on perinatology and neonatology and laid the foundations for a partnership in pediatrics between the National Medical University and the University of Pennsylvania (Philadelphia, USA), which continues today.

Victor Mikhailovich has prepared a bright galaxy of pediatric scientists – 8 doctors and 23 candidates of medical sciences. Among the students of Viktor Mikhailovich Sidelnikov: prof. O.I. Lasitsa, prof. Kazak S.S., M.D. Makarenko M.O., prof. Peshiy M.M., prof. Kuzmenko A.Y., Corr. Member of NAMS of Ukraine prof. Volosovets O.P., prof. Kryvopustov S.P., M.D. Rudnev O.M. etc.

In 1997 after early death of V.M. Sidelnykov the head of department became his pupil professor Oleksander Petrovych Volosovets (born on 26.09.61). He is a corresponding member of the National Academy of Medical Sciences of Ukraine, heard of commission on Content of Education and Standardized Quality Assessment of Training of Medical Specialists and Pharmacists  of the MHC of Ukraine, Deputy Head of the Scientific Medical Council of the MHC of Ukraine, Vice-President of the Association of Rheumatologists of Ukraine, Advisor to the Head of the Association of Pediatricians of Ukraine, Honored Worker of Science and Technology of Ukraine. He is high-achiever of education. He was awarded the Diploma of the Presidium of the Academy of Medical Sciences of Ukraine  and Acad. Strayhesko medal for Health Care Services (2009).He is a laureate of Dr. Requiega Ukrainian Prize (2005).

He has about 850 scientific works, 17 patents for inventions, 8 textbooks,  61 study guides and 12 monographs. His scientific works are devoted to problems on research of impact from consequences of Chernobyl accident on children’s organism, diagnostics and treatment of inborn and acquired injuries of myocardium at children, development of the newest diagnostic approaches in children’s cardiorheumatology, allergology and neonatology. Volosovets O.P. in his doctoral dissertation “State of cardiovascular system at children from areas of ecological ill being” proved the impact of radionuclides and other ecotoxic factors of contaminated environment on development of acquired injuries of the heart at children that suffered as a result from the Chernobyl accident and developed the ways for complex pathogenic therapy of those states. The separate direction in the medicine of childhood – ecological pediatric cardiology was established and continues to develop and now.

Over the past twenty years, 13 candidates and 2 PhDs have been trained at the department. The staff of the department has performed a large series of works on neonatal cardiology, pediatric pulmonology, allergy and gastroenterology, implemented modern ultrasound diagnostic methods in the practice of health care. These developments formed the basis for the 1999 and 2016 textbooks: “Children’s Diseases”, “Medical Genetics” (2010), “Pre-hospital and early hospital care of children” (2019), and a number of study guides.

The research of the Department of Pediatrics No. 2 formed the basis of the State Protocols for the Treatment of Cardiovascular Diseases in Children. Head of the department Volosovets O.P. jointly with leading specialists from other institutions of higher medical education, developed, scientifically based and implemented a system of training specialists for health care, witch is based on the principles of unified continuous training, retraining and skill upgrades for specialists, management of quality of their preparation. Was organized a series of scientific and practical seminars on the continuous professional development of pediatric and family doctors “Problem-oriented pediatrics” in Ukraine.

The educational and methodological work of the department is focused on the following important directions of formation of the future pediatric and family doctor:

  • the basis of educational programs in all courses of teaching – achievement of quality of training of doctors in the specialty “Pediatrics” and “Medicine” in the undergraduate and postgraduate stages
  • mastering in the process of preparation by future doctors of modern international protocols of treatment and diagnosis of common childhood diseases
  • Practically oriented training of future doctors
  • implementation of new pedagogical and medical technologies
  • systematic control of the level of learning.

A modern type of practically oriented exam at the Bogomolets National Medical University is an objective structured clinical exam (OSCE) for the objective assessment of such important competencies of the future doctor, such as communication with the patient and parents of the child, ethics, clinical examination and technique of performing medical procedures, including invasive interventions. The department has developed a modern methodology for conducting exam “OSCE 2” at a pediatric station, devoted to the method of providing emergency care to a newborn baby. Assistant Professors Loginova I.O. and Shevtsova T.I. prepared the corresponding training videos “Newborn Examination” and “Newborn Resuscitation”.

In the senior years of study at the Pediatric Diseases Clinic, the department makes particular emphasis on the relevance of teaching pediatric illness issues, bringing together thorough theoretical and permanent practical training for the future doctor at the clinic.

The achievements by collective of department were recognized by Ministry of health care of Ukraine, which approved the department as supportive one among pediatric departments in higher schools of III-IV accreditation levels.

The staff of the department under the direction of O.P. Volosovets has in recent years implemented into the health care a number of new methods of diagnosis and treatment, in particular: assessment of coronary circulation in children using Dopplerography; early diagnosis and treatment of heart failure in children; treatment of diseases of the bronchopulmonary system in children; modern methods of diagnosis of severity of atopic dermatitis and bronchial asthma in children.
