History of the department

The department of dental surgery was founded in 1920 and named “the department of operative teeth care”. It was headed by practicing doctor-dentist M.J. Shapiro who in 1901 undertook an internship in Berlin during four months in specialty “orthognatic surgery”. The classes were held on the basis of surgery at dental polyclinic, the teachers (chairman and 3 teaching assistants including S.N.Vaysblat) provided only academic process. Clinical site of the department was situated at the building of dental polyclinic in Khreschatyk Str., 50, and the curriculum of the department contained two clinical disciplines – operative teeth care and surgery of oral cavity and adjacent areas. Professor M.Y. Shapiro headed the Department to 1931 and then it was headed by professor S.N. Vaysblat, the student of chairman department of intermediate level surgery of Kyiv Medical Institute professor N.A. Zavyalov. At the same time, S.N. Vaysblat concurrently headed the department of dental surgery at Kyiv institute of doctors’ advancement. Under the guidance of S.N. Vaysblat, in Ukraine was created the scientific school of dental surgeons and maxillofacial surgeons.

In 1929 there were founded 4 beds for maxillofacial patients in the clinic of intermediate level surgery of 1st municipal hospital and in 1931 there was opened the maxillofacial division in Kyiv institute of traumatology and orthopedics, at first for 16 beds, and since 1937 – for 40 beds. In-patient facility was situated in park above the Dnipro in Kirov Str., 7, at light and good three-storied building (today it is the residence of Ministry of Public Health of Ukraine). In 1935 there was founded dental technical laboratory, affiliated to the clinic, for production of dental splints, complex prosthetic dentures and appliances. The clinic carried out the reception of outpatients and at ambulance too. Clinical sites were transferred to government funding and consigned to the institute, and the students were prohibited to attend the lectures freely.

Maxillofacial division of Kyiv institute of orthopedics and traumatology was the clinical and scientific site of the department where a great volume of treatment and scientific work was performed; it was mostly dedicated to two problems: local anaesthesia of maxillofacial area and odontogenic osteomyelitis of jaws. In 1925 there was published a monograph of S.N. Vaysblat “Nerve block anaesthesia of teeth and jaws” which was the first monograph dedicated to local anaesthesia in dentistry and eventually had 6 editions, and in 1938 there was published the monograph of S.N. Vaysblat “Purulent osteomyelitis of jaws”.

In course of time the training period at the faculty was extended up to 5 years and the disciplines of dental and maxillofacial surgeries were taught not 2 but 3 years. Before the beginning of war, the personnel of the department had the following fifteen posts: chairman, three associate professors, eleven teaching assistants, ten resident physicians, seven post-graduate students and four denturists.

In total, under the guidance of professor S.N.Vaysblat, there were executed 12 candidate dissertations. He wrote about 120 scientific works including monographs: “Nerve block anaesthesia of teeth and jaws” which had 6 editions and “Purulent osteomyelitis of jaws”. Both works still have great theoretical and practical value.

Being the member of Kyiv dentists’ association (1935–1953) and the head dentist of Ministry of Public Health of the Ukrainian Soviet Socialist Republic (1946–1953), professor S.N. Vaysblat made a lot for scientific and practical training of dentists in Ukraine. He took part in organization of regional dentists’ associations, was the initiator of implementation of new scientific achievements into practice of treatment institutions. S.N. Vaysblat was awarded the title of Honored Science Worker of the Ukrainian Soviet Socialist Republic.

In March 1953, immediately after professor S.N. Vaysblat’s retirement, the department was temporarily headed by the participant of Finnish and Great Patriotic Wars – associate professor G.I. Semenchenko (graduated from Kyiv Medical Institute in 1938) and then the department was headed by Fetisov Mykola Vasylyovych.

In March 1953, immediately after professor S.N. Vaysblat’s retirement, the department was temporarily headed by the participant of Finnish and Great Patriotic Wars – associate professor G.I. Semenchenko (graduated from Kyiv Medical Institute in 1938) and then the department was headed by Fetisov Mykola Vasylyovych, 1896 year of birth, participant of the Civil War, graduate of the treatment faculty of Kuban medical institute of 1925. He headed the department in 1953–1961 yy. In 1940 M.V. Fetisov defended his candidate dissertation (“Materials before the state of oral cavity among brucellar patients”), in 1956 he defended doctoral dissertation (“Variants of operative approach to infratemporal fossa”) and in 1956 received the title of professor.

During these years the main areas of scientific work were: further development of anesthetic problems in maxillofacial area, odontogenic osteomyelitis (V.S. Kovalenko, A.M. Korolenko), operative approaches to infratemporal and pterygomaxillary fossas and questions of plastic surgery. There were offered the methodologies of nerve block anaesthesia and methods of nose malformations removal upon schistasis of upper lip, formation of bucket-handle graft with bone defect replacement of lower jaw, osteosynthesis of lower jaw fractures with the help of wire suture, metal core or capron thread, filling of cystic cavities with homo- and heterotransplants (N.S. Koval). Under the guidance of M.V.Fetisov there were defended five dissertations (D.I. Tsentylo, N.S. Tsebenko-Guliyeva-Koval, E.I. Bocherikova, Y.G. Semenenko and A.M. Korolenko).

In 1961 р. professor V.M. Fetisov was transferred to the post of department’s chairman of dental surgery in Kishinev medical institute. To head the department in Kyiv medical institute it was supposed to invite professor E.S. Malyevych (Dnipropetrovsk) who was the candidate of medical science and worked as a teaching assistant of the department just before the Great Patriotic War; in hospitals he received a great treatment experience of military firearm maxillofacial wounds. But it did not happen due to a number of reasons. Eventually the department was headed by professor Y.Y. Bernadskiy.

In such favorable conditions the effectiveness of department’s work increased. In 1965, 42 persons worked at the department, among them 13 were teachers: chairman (professor Y.Y. Bernadskiy), 3 associate professors (S.I.Bukh-Chechyk, V.S.Kovalenko, V.A.Petrenko), 9 teaching assistants (E.D.Pokotylo, L.V.Nazarenko, N.S.Koval, E.I.Bocherikova, P.V.Khodorovych, E.V.Odynets, I.O.Raksha, V.I.Kozlyuk, R.N.Chekhovskiy), 5 post-graduate students, 5 resident physicians, 4 polyclinic doctors, 5 laboratory assistants and other personnel.

Under the guidance of professor Y.Y. Bernadskiy, there were studied history and bibliography of soviet dentistry, pathophysiology of operation trauma, traumatology, oncology, restorative-reconstructive surgery of maxillofacial area and gerontological dentistry. There were received new data on pathophysiological changes upon maxillofacial operations. There developed premedication methodologies and anaesthetic methods with application of neuroleptic and tranquilizing medications; there was recommended new anaesthetic drug – trimecaine and there was implemented intratrachaeal anaesthetic. In 1981 there was started the study of general anaesthesia among children.

In 1965 professor Y.Y. Bernadskiy was the first who published in Ukrainian the study guide “Surgical dentistry” (Kyiv, 1965, 265 p.), in 1970-1972 there was published 2-volume guidance by Y.Y. Bernadskiy “The fundamentals of surgical dentistry” and “Traumatology and restorative surgery of maxillofacial area” which became indispensable guides practically for all specialists of the Union of Soviet Socialist Republics and for more than 30 years , they had 4 editions with general circulation of 40 thousand copies.

In 1975-1985 there were published methodological recommendations (29 parts) to practical classes for students and teachers in all sections of specialty which were extended not only in Ukraine but all over the USSR. At the department there were qualification courses for teachers of medical institutes of the country – professors, associate professors, teaching assistants who at the same time with education took part in all kinds of department’s activity, shared their scientific, teaching and treatment experience.

The history of soviet dentistry and bibliographic analysis of scientific achievements of dentistry schools of the USSR were reflected in two volumes of bibliographic guidance “Soviet dentistry literature”. At the department, there were written about 20 monographs, textbooks and guides, among which were study guides for students of dentistry institutes and faculties of “Surgical dentistry” (1966), “The fundamentals of dental surgery” (1970, 1983, 1984, 2003), “Traumatology and restorative surgery of maxillofacial area” (1978, 1985, 1999) by professor Y.Y. Bernadskiy, monographs “Odontogenic maxillary sinusitis” (1968), “Gerontological dentistry” (1973), “Outlines of purulent maxillofacial dentistry” (1978), “Rehabilitation of children with nonunion of palate” (1978) and others.

Effective work was encouraged by convenient clinical sites of the department – maxillofacial division of the hospital № 23 for 65 beds which was situated on 4th floor of the dental clinic and on 1st floor – outpatient surgical division, first for 18 and since 1983 for 36 dental chairs. Lecture room was situated on 1st floor that is why every lecture was conducted with presentation of 1–3 subject patients who were taken by lift from 4th to 1st floor to the lecture room. Patients were presented to students by post-graduates, resident physicians and doctors of the division. Therefore, critically ill patients could be immediately lifted into division and given an emergency care including with the participation of students. All this was positively reflected on the quality of specialty teaching and on quality of students’ training.

In 1977 by the recommendation of professor Y.Y. Bernadskiy and professor S.Y. Kryshtab, there was resumed the application of dental implantation method in dentistry (V.O. Malanchuk, V.V. Los’).

In 1980 at children’s hospital № 3 was founded children’s maxillofacial division for 30 beds; it was headed by associate professor R.N.Chekhovskiy. In 1983 this division was transferred to the hospital “OHMATDIT”, the number of beds was increased up to 60 and it was headed by associate professor J.V.Dudko.

1989 to October 1993, the department was headed by professor J.V.Dudko who simultaneously worked as a dean of dental faculty. At that period the department’s staff amounted to 17 rates: chairman – 1; consultant professor – 1; associate professors – 3; teaching assistants – 12.

At that time (in 1989) at the clinical site of the department – maxillofacial division, was closed the emergency station which worked 15 days per month and where students worked during practical classes and makeup of missed classes, there were reduced 4 doctoral rates (doctors were transferred to municipal medical emergency hospital and then fired out). It significantly worsened the training of students and young specialists, as well as teaching, treatment and scientific work of the department.

The beginning of 90-s distinguished by negative phenomena which reflected the real social situation during difficult period of Ukrainian nation-build. During this period, due to the emergence of new socio-political, economic and moral realities that significantly worsened the quality of human life and forced them to think about the work in order to “survive”, and significant deterioration of the department’s clinical site, there almost completely stopped the influx of young doctors-enthusiasts to the department. During this time (1989–1993) at the department was defended only one candidate dissertation executed in medical emergency hospital of Kyiv city with the help of neurosurgeons.

Maxillofacial division started to provide only planned surgical service what was quite a little for full-scale training of medical personnel, future department’s teachers; also it negatively affected the care quality to patients, the qualification level of most clinic and department workers, training of scientific personnel, scientific and teaching work, etc.

In October 1993 professor J.V.Dudko died and from 24.11.1993 to 31.01.1995 the liabilities of department’s chairman were performed by associate professor L.F.Poznyak (head teacher of the department).

Since 1.02.1995, the department’s chairman became Doctor of medical science, associate professor V.O.Malanchuk. On 29.12.1994 he defended doctoral dissertation in the following subject: “Reconstructive-restorative operations of lower jaw (clinical laboratory research)” where different complex operations of lower jaw were pathogenetically and biologically grounded. For the first time, on tissue culture the author demonstrated that lower jaw has various restorative capacities upon dissimilar pathological processes. There were developed methods of distractive and microsurgical removal of defects and malformations of lower jaw (since 1985); treatment methods of ankylosis of maxillo-temporal joint by means of patient’s metatarsophalangeal joints transplantation in a form of free or vascularized transplant (the first operation in the USSR, 1986); there was grounded the choice of pathogenetic-optimal free autologous bone transplant for local correction of restorative osteogenesis being aberrant upon diseases of lower jaw’s bone tissue.

From 1994, the emergency station, as part of maxillofacial division, was restored; the number of doctoral rates in maxillofacial division was increased from 3 to 7, but the duty days became only two days per week which was extremely insufficient for pedagogical process and fill up the beds with patients, the number of which has reduced to 50. Gradually, there was started the restoration of the volume and level of division’s clinical work which had reduced due to five-year absence of patients (in 1989–1994 yy.) and who are admitted through emergency station by medical ambulance.

Expansion of scientific contacts enabled the department to create in 1995 a new educational program for students of higher medical educational institutions of Ukraine – the department started to teach the specialty according to new program developed by it; this program included 15 new topics and chapters according to modern educational programs of European countries and major modern achievements of world’s professionals, specialty’s position in the world (shreds with axial vascular pattern, microsurgery of maxillofacial area, compression distraction method, maxillofacial oncology, reconstructive-restorative surgery and others). After successful testing of the new program by the department in 1998 with participation of colleagues from Kharkiv and Poltava, this program was specified and approved by the Ministry of Public Health of Ukraine. Since then all Higher Medical Educational Institutions started to work according this new program and the students of dental faculties started to learn a new complete modern course of specialty.

Thus, the department raised to a new modern level the quality of specialty teaching in Ukraine; there was formally completed the first step of contingence and unification of national, European and world professionals training system in the field of dentistry and, especially, maxillofacial and cranio-facial surgery; there was made the main step towards contingence of specialty’s position and its teaching to the level of developed countries of the world; access of Ukrainian medicine to world’s medical space. Over the years, some of the most difficult parts of this program, in particular, on reconstructive-restorative and cranio-maxillofacial surgery, were reviewed (2006) in European association of cranio-maxillofacial surgeons and received high appraisal and were amounted to be one of the best in Europe.

Due to increase of students number, the department started to increase either and there was felt the shortage of teaching personnel. In 1995 young teachers joined the department – 2 masters, 8 interns and in a year they were already 16. They started to study according to new educational programs developed by the department, including related departments and in units of general surgery, children’s maxillofacial surgery, neurosurgery, traumatology, otorhinolaryngology, otorhinolaryngology-oncology, oncology of head and neck, microsurgery, ophthalmology and other related specialties (about 1-2 weeks).

There were developed international ties of the department. During the visits to the department in 1992–1993 yy. of professor R.Fris (Linz, Austria), the President of European association of cranio-maxillofacial surgeons in 1990-1992 yy., there was founded the organizing committee on foundation of Ukrainian association of cranio-maxillofacial surgeons.

On 12.01.1996 the department held the second organizational conference (it was prepared since spring 1995) with participation of departments’ chairmen of surgical dentistry of Higher Educational Institutions of Ukraine, heads of maxillofacial divisions of hospitals, famous specialists, and according to their decision there was created the Ukrainian association of cranio-maxillofacial surgeons (UACMFS).

Participants of the conference discussed the regulations of European similar association and Association of Ukrainian dentists in comparative aspect; there were adopted necessary foundation documents; there were chosen governing bodies of Association and others. Its President became the Doctor of medical science, associate professor V.О. Malanchuk, vice-president – professor L.V. Kharkov, honorary president – professor Y.Y. Bernadskiy. Among founders and honorary members of Association are professors, associate professors and doctors of all institutes and regions of the country, well-known foreign maxillofacial surgeons of Europe, America, CIS and famous Ukrainian specialists of other professions. Foundation of Association became the continuation of ties development of Ukrainian cranio-maxillofacial surgeons with foreign colleagues, in particular with European Association of cranio-maxillofacial surgeons (EACMFS).

At the end of December 1999, due to closure (as if temporarily) for repairs of clinical site of the department – hospital № 14 (former – № 23 n.a. М.І.Кalinin, Zoologichna Str., 3), maxillofacial unit was reduced from 50 beds to 30 and transferred (according to promises, as if temporarily, during the repairs) from 1.01.2000 to Kyiv municipal clinical hospital № 12 (Prof. Pidvysotskiy Str., 4-а) in Pecherskiy district where was already situated similar maxillofacial unit № 1 for 60 beds. Unfortunately, the hospital is located at a long distance from Zoologichna Str. – the main outpatient site of the department (more than one hour driving distance by public transport).

In the hospital № 12 only 3 classrooms and 2 more – for staff, with a total area of ​​65.5 m2 were provided to the department. The department has only one operating room, designed for one operating table (but there are two operating tables), one dressing room (first you need to bandage “clean” and then – “purulent” patients, which does not meet the requirements of the pedagogical process), admission department – general with a similar maxillofacial department №1 – clinical base of the department of Shupika NMAPE. The X-ray department does not have the necessary equipment for examination of maxillofacial patients, anesthesiologists of the hospital do not fully know the methods of anesthesia of maxillofacial patients, so these doctors were trained in our department, there were other difficulties.

In the spring of 2002, the city authorities planned to reduce the maxillofacial department of the department by 10 beds (up to 20 beds) with the loss of the status of an independent department. The maxillofacial department was to be attached to the 60-bed department, on the basis of which the Department of Maxillofacial Surgery of Shupika NMAPE. As a result, the department would lose its clinical base, and only due to timely information about the situation of the rector of the Bogomolets National Medical University Academician E.G. Goncharuk and his intervention in these processes managed to prevent the wrong development of events and prevent the loss of the department of its main clinical hospital base.

All this significantly worsened the working conditions of the department, but the educational process continued. Due to the fact that after 1995 the number of students at the faculty began to increase and at the same time the number of teachers increased, the department began to experience a shortage of premises for the educational process. Therefore, in accordance with the agreement between the University and the Directorate of the Institute of Oncology of the Academy of Medical Sciences of Ukraine (director – Prof. S.O. Shalimov) since 2000 the department has received permission to conduct not only 1-2 thematic classes on maxillofacial oncology with students of dental and medical faculties. on the basis of the Department of Head and Neck Tumors and methods that modify treatment (head – Honored Worker of Science and Technology, Professor V.S. Protsyk), but also constantly work with 1-2 groups of students in the clinic of the institute.

At the same time as private outpatient clinics began to be used private dental clinics “Renaissance” (until 2006) and “Cheremosh” (until 2010), the heads of which V.F. Tokarsky and V.Y. Grabovetsky received good professional training during the previous work as doctors of the maxillofacial department and assistants of the department, and in difficult times for her provided opportunities for their clinics to ensure the pedagogical process. At the same time, they also worked as assistants at 0.25-0.5 rates.

In 1995, the department had 17 rates of teachers, in 1999 the staff of the department consisted of 26.75 rates of teachers and 9 rates of support staff, in 2001 – of 30.5 rates of teachers and 14 rates of support staff, in 2002 – from 31.75 rates of teachers, 2005–2008 – from 32 rates of teachers (3.5 of them – for interns). At this time, the department simultaneously studied 6 clinical residents (including 3 foreigners), 10 interns, 3 graduate students (2 – foreigners), the department continued to teach the course “Fundamentals of Dentistry” to students of 4 medical faculties (over 1200 people).

It was urgent to prepare new staff for the department and the clinic. Therefore, in 1995-1996, 6 graduates of the faculty were invited to the department – 4 interns for 2 years of training, who later filled the vacancies in the hospital and clinic, and 2 masters (V.V. Protsyk, O.L. Serenkova), who defended their master ‘s theses, entered the postgraduate course 2 years later, defended their dissertations and became teachers (assistants).

In 1997, 20 young doctors studied at the department: 2 graduate students, 2 clinical residents and 16 interns in 2- and 1-year training programs. In the same year, the Ministry of Health of Ukraine, unfortunately, with the consent of the head of the Department of Maxillofacial Surgery NMAPE reduced the period of study in the internship from 2 years to 1 year, but at the same time allocated for general and orthopedic dentistry for 1.5 years in the internship!

On September 25, 1995, the Institute of Neurosurgery of the National Academy of Medical Sciences of Ukraine (Director – Academician of the National Academy of Medical Sciences of Ukraine Yu.P. Zozulya) together with neurosurgeons performed the first planned craniofacial surgery to remove a benign brain tumor from access through the mouth and nose after osteotomy and breeding in the direction of the upper jaws. Later, other new methods of removing intra- and extracranial tumors began to be developed. In the same years, employees of the department were invited to participate in operations at the Research Institute of Otorhinolaryngology NAMS of Ukraine, including – in the oncology department to remove common tumors of the ENT organs and neck with neurosurgeons, which significantly increased the effectiveness of treatment of this group.

Scientific and practical achievements of the staff of the department led to the fact that in 1996 and 1997 the head of the department V.O. Malanchuk participated in the work of state commissions for the identification of Ukrainian citizens who died in plane crashes in Svalbard (August 28, 1996, Norway) and in Thessaloniki (December 20, 1997, Greece), for which he received a letter of thanks from the Minister of Health as part of a group of forensic experts. I of Ukraine. From Norway and Greece V.O. Malanchuk organized humanitarian aid: in 1997 – from the University of Oslo (Norway) received a dental unit, in 1998 from the Municipal Hospital of Athens (Greece) received a dental unit, defibrillator, anesthesia machine, 2 functional beds, 2 liquid infusion systems, in 1988 – consumables (syringes, bandages, etc.) were obtained. The total cost of assistance received – up to 80-100 thousand USD.

December 25, 2002, the head of the department, Professor V.O. Malanchuk, according to the competition, was enrolled as the head of the Department of Health, Family, Youth, Physical Culture and Tourism of the Humanitarian Policy Department of the Main Department of Internal Policy of the Administration of the President of Ukraine, where he worked until April 15, 2005. A number of drafts of important documents on humanitarian policy of the state, became a civil servant of the 5th rank. In the spring of 2003 he managed to keep 5 offices (with an area of ​​about 120 sq.m.) on the 4th floor of the dental clinic of the university, where repairs were started, and in 2004 – early 2005, 4 new dental units were installed there. In January 2000, the Department held an international conference on the occasion of the 85th anniversary of professor Y. Bernadskiy and the 80th anniversary of the department foundation (and dental faculty). The materials of the conference were published as an individual book “The Department at the turn of Millennium”, which also includes the essays about the faculty and department history about the biographies of department heads and professors, and other informative materials which were prepared for the celebration of 160th anniversary of the Bogomolets National Medical University in 2001.

In 1996 the department started the collaboration with Ukrainian Ministry of Public Health as for the introduction of medical specialties in Ukraine which are actually existed as a profession – ”Oral and maxillofacial surgery”, because the department specialists and more than 500 oral surgeons of the country, and all dentists who should perform surgeries on the neck (do tracheotomy, tie carotid artery, reveal phlegmon) are working outside the legal field. On the 25th of May 1999, the Department filed a third application (№ 22\124) to Ukrainian Ministry of Public Health with a request to include into the list of medical specialties a new one – “27.6 – maxillofacial surgery”, which has been confirmed and transferred to the Ministry together with the documents from 20 regional offices of health care and 7 medical universities of Ukraine. And the essence was the following (further – the letter to the President of Ukraine for the purpose of the statement of a specialty “maxillofacial surgery” in Ukraine).

The current situation in the highly specialized medical specialty of surgical dentistry and maxillofacial surgery, which employs about 1,400 specialists in Ukraine, makes me turn to you (V.A. Yushchenko – ed.) With the following.

Thus, the specialty of surgical dentistry involves operations only in the oral cavity, and maxillofacial surgery – including on the face, neck, taking parts of the ribs, iliac bone, bones and joints of the foot for transplantation to the jaws and more. In each regional hospital there is a maxillofacial department (there are as many as 6 of them in Kyiv!), There are already departments of medical universities, and the profession itself does not legally exist! Therefore, a doctor in any case, whether or not to operate on the face, neck, even providing assistance on vital signs, still violates the law and is subject to legal liability: in the first case for going beyond the scope of his professional responsibility, and in the second – for failure to provide assistance to the patient.

In Russia, the specialty of maxillofacial surgery was introduced in 1995. From the same year, the department raised before the Ministry of Health the introduction of the same specialty in Ukraine, and the creation of the Ukrainian Center MFS. The initiative was supported by the Cabinet of Ministers of Ukraine. According to the order of the Ministry of Health of Ukraine № 9 NDM from 09.01.2000, a working group of 5 professors submitted to the Ministry of Health the necessary documents to create a specialty (№22 / 59 from 09.03.2000), and it all ended because the change of government and instructions of the former the government did not comply.

However, since 1992, NMU specialists have represented Ukraine in the Council and Confederation of the Association of Craniofacial Surgeons, the International Association of Maxillofacial Surgeons. In 1996, the Ukrainian Association of Cranial and Maxillofacial Surgeons was established and operates, in 1996 and 1997, Ukrainian specialists duly fulfilled the instructions of the state, working in government agencies to identify those killed in a plane crash in Norway (Svalbard) and Greece. Thessaloniki). Internships in clinics of developed countries were held by about 10 specialists of Ukraine, reports of scientists of Ukraine were constantly presented at international scientific forums.

Since 1994, Ukraine has been participating in the European and world process of unification of physicians, training and treatment of patients, there are materials on the state of affairs in the field of surgical dentistry and maxillofacial surgery included in the so-called “White Paper” (1994). .

The high scientific interest of foreign experts led to the fact that UACMFS twice received an offer to hold in 2007 and then in 2009. World Conferences of the International Association of CMFS, but in the current absence of a profession and a specialized center, we were forced to twice refuse this offer.

Now, for the third time, the same offer has been received for 2011, but there is still no profession, no state (city) center.

The current Minister of Health of Ukraine M.E. Polishchuk knows this problem. Back in May 1987, we operated on a patient with an extremely severe craniofacial trauma, and in 1993 we agreed on the further development of joint work.

Obviously in the interests of the state:

– to introduce the specialty of maxillofacial surgery

– to create a state center for MFS as a clinical base of the main department of the Ministry of Health of Ukraine (at the Department of Surgical Dentistry of the National Medical University)

– provide support in the development of international relations of Ukrainian specialists with foreign specialists.

The best place to create a center is the former building of the burn center at 3 Zoologichna Street, which has a lecture hall for 150 people (near the Institute of Otorhinolaryngology), or to add 4 and 5 floors of the old building of the ENT Institute (at the same address). working conditions of the main department of Ukraine, in which she worked from 1963 to 2000.

Moreover, this issue was discussed three times only in 1999 with the Minister of Health of Ukraine (verbal consent was obtained, including in the presence of the Deputy Prime Minister of Ukraine for Humanitarian Affairs on September 30, 1999) and the First Deputy Minister.

On December 29, 1999, the department received the support of the Cabinet of Ministers of Ukraine (separate order of the Cabinet of Ministers of Ukraine №28531 / 96) on the introduction of the specialty “maxillofacial surgery” in the state. Thanks to this order, by order of the Ministry of Health of Ukraine (№ 9-ADM dated 19.01.2000), a working group of 5 heads of departments (Professor V.I. Vakulenko, Odessa; MD V.O. Malanchuk, Kyiv, Professor O.E. Malevich, Dnipropetrovsk, Professor O.V. Rybalov, Poltava, Professor G.P. Ruzin, Kharkiv) developed and submitted in March 2000 to the Ministry of Health of Ukraine a package of necessary documents. But since then, numerous appeals to the Ministry of Health and other institutions (including the President of Ukraine in June 2009) have failed. The issue of introduction of the specialty “maxillofacial surgery” and the creation of a state or city center of craniofacial surgery as a clinical base of the department, the proper equipment of the clinic with modern equipment could not be solved.

However, even in such difficult conditions, the department continued its comprehensive active work. Thus, in very difficult organizational conditions (the smallest areas per 1 student among dental departments; the presence of only 9 classrooms for 14-20 groups of students who are simultaneously at the department; the territorial gap of the department; the transfer of student internship in 1994 from winter to summer, etc.) the department conducted pedagogical work with the largest contingent of students among the profile departments – 3 courses of the Faculty of Dentistry (about 1200 students), 3 medical faculties (1, 2 and 3 – about 1200 students), military faculty (50 students), doctors- interns (400-450 people) – that is, only 2600-2700 students.

Testing students’ knowledge, especially at the stages of State certification, was and remains a responsible moment in the pedagogical process of the department, determining the level of theoretical and practical training of future dentists.

Over the years, the department had the top at the University of inventive work, constantly attracting young people to scientific work. Forms of work with young people are different – research students circle the shifting of students in the emergency department of a clinical site, presence at surgeries, reports on current topics that yield positive results. Thus, in 1999, for the first time of the NMU history the student and the member of the department’s circle A.V. Kopchak won the Republican contest for the best student work, in 2001 won the competition for the best scientific work of young scientists of the country, and in 2003 he defended a dissertation on ozone-oxygen therapy.

In 2009 and 2010 the Department together with the Departments of eye diseases (Head – Corresponding Member of the National Academy of Medical Sciences of Ukraine, Prof. G.D. Zhaboyedov), the Department of Otorhinolaryngology (Head – Prof. Y.V. Mitin) and the O.A. Kisel Student Scientific Society held two interdepartmental youth scientific-practical conferences, which presented about 30 reports of 2nd-6th year students, clinical residents and graduate students of the departments. Circle members spoke at foreign conferences and forums: Kosice (Slovakia, March 2-9, 2014), where 5th year student Julia Hryb took first place (works at the department), Batumi, Georgia, July 11-13, 2014), Dubrovnik (Croatia, August 17-24, 2014) and All-Ukrainian conferences with international participation (Uzhhorod, Ternopil, Kyiv, etc.), as well as held various workshops.

The effectiveness of the department is confirmed by the publication in 2000–2003 of the first and fifth editions of the two-volume manual of Professor Yu.Y. Bernadsky “Fundamentals of Surgical Dentistry and Craniofacial Surgery” and “Traumatology and Reconstructive Surgery of the Craniofacial Region” in Russian and Ukrainian, and increasing the number of students at the department – citizens of other countries, and defense of dissertations. At the end of 2000, Professor Yu.Y. Bernadsky became seriously ill and retired from active work at the department.

In 2009, the team of authors of the dental faculty of NMU and the National Dental Academy (Poltava) on behalf of the Ministry of Health of Ukraine published a textbook “Fundamentals of Dentistry” (with the appropriate stamp of the Ministry of Health) for medical students of higher medical institutions of Ukraine (editor – Professor V.O. Malanchuk).

In 2011, the department published a textbook “Surgical Dentistry and Maxillofacial Surgery” in 2 volumes, which covers current issues of surgical dentistry, the achievements of the department and the latest methodological and operational approaches in craniofacial surgery. The material presented in the textbook corresponds to the new program “Surgical Dentistry” and “Educational and qualification characteristics of the specialist.” Recommended by the Ministry of Education and Science of Ukraine as a textbook for students of HMEI III-I V levels of accreditation.

During 1999–2015, 4 doctoral dissertations were defended by researchers and employees of the department – V.G. Centilo (Donetsk, 2007), Ya.P. Nagirny (Ternopil, 2009), OS Volovar (Kyiv, 2013), A.V. Kopchak (Kyiv, 2014) and 10 candidate dissertations: NO Dovbish (1999), O.S. Volovar (2000), O.O. Astapenko (2001), VV Protsik (2001), A.V. Kopchak (2003), O.L. Tsilenko (Serenkova) (2004), I.P. Logvinenko (2005), A. Moddares (Iran, 2007), MM Chumak (2007), M.A. Gordiychuk (2010), VM Yefisko (2011), I.S. Brodetsky (2011), TV Good Evening (2011), I.V. Fedirko (2011), Yu.V. Chepurny (2011), I.G. Skvortsova (2012), I. Farshad (2013), N.M. Timoshchenko (2015). Most of the topics of the dissertations of these authors were devoted to the most complex section of the specialty – reconstructive and reconstructive surgery of the facial skull and its biological aspects.

The scientific achievements of the department led to the fact that in 2009 for the first time the head of the department, Professor VO Malanchuk was invited as an official opponent to defend a dissertation on craniofacial trauma in “neurosurgery” (at the Institute of Neurosurgery of the National Academy of Medical Sciences), and in the spring of 2010 – an opponent of his dissertation on sinusitis treatment at the Institute of Otorhinolaryngology.

The organizational work of the department also deserves attention. In January 1998, the first Republican Conference “Modern Dentistry and Maxillofacial Surgery” was held, which was attended by about 300 participants (published abstracts). -anniversary of the professor-consultant of the department Yu.Y. Bernadsky. As a result of her work, the book “Department at the turn of the millennium” was published.

In May 2004, 2006, 2008, 2010, 2012, 2014, the department and the Ukrainian Association of Cranial and Maxillofacial Surgeons held international congresses on dental implantation, which were attended by more than 2,000 representatives from 24 countries. Presidents of Ukraine LD Kuchma (2004) and V.A. Yushchenko (2006), Chairman of The Verkhovna Rada of Ukraine V.M. Lytvyn (2010) and other heads of state welcomed participants in these international scientific events. In 2009, 2011, 2013, the department held a Congress of the Association of Craniofacial Surgeons.

In January 2008, the department held the first nominal scientific-practical international conference dedicated to the 120th anniversary of the birth of Professor S.N. Weissblatt, in January 2009 – a similar conference on the occasion of the 95th anniversary of the birth of Professor G.I. Semenchenko, in January 20 11 – Conference “Modern Dentistry and Maxillofacial Surgery”, dedicated to the 90th anniversary of the birth of Professor V.S. Kovalenko, January 2012 – a conference dedicated to maxillofacial surgeons who took part in the Second World War, January, 2013 – a similar conference dedicated to veterans of the Second World War, O.M. Korolenko and N.S. Koval, employees of the Department of Surgical Dentistry and Maxillofacial Surgery of NMU named after OO Bogomolets, and on February 30, 2015 the final thematic conference dedicated to the 100th anniversary of the birth of Professor Yu.Y. Bernadsky. Extremely painstaking and in-depth work was carried out to study the life and scientific path of professors, maxillofacial surgeons, and their contribution to the development of the specialty.

Scientific materials of all these events were published in separate books, sent to all medical libraries of Ukraine, central libraries of the CIS countries and leading professors of maxillofacial surgeons of the CIS countries and received their high praise.

The significance and unusualness of the maxillofacial area, not only as a complex anatomical and topographic area, but also the passport part of the dental and maxillofacial system of the body, a factor of psychological instability of the patient is undeniable. And not without reason the well-known scientist, professor Yu.Y. Bernadsky: “You can’t cover your face with pants”, on the 100th anniversary of the birth of which the department dedicated a recent thematic conference, which took place on November 30, 2015.

The memory of the teacher, colleague, and outstanding maxillofacial surgeon has been restored for years in his memoirs by the dean and heads of the departments of the dental faculty, library staff, and all comers. During the conference there was an exhibition of scientific works and the work of Yu.Y. Bernadsky. The event received a positive assessment among employees of departments and doctors of the Dental Medical Center of the Bogomolets NMU in terms of educating young people, following the principles of morality, discipline, devotion to a favorite cause. And it is extremely significant that the department pays tribute to the historical value, the experience of generations in the form of respect for professors, veterans, surgeons and the 6th scientific-practical conference “Modern Dentistry and Maxillofacial Surgery” from the series of thematic events.

At the same time (since 2008) due to the decrease in the number of patients in the outpatient clinic, the department organized television broadcasts from the planned operating room to the classroom with two-way communication between the surgeon and students.

Today, thanks to modern technology and information and technological progress, it is now possible to record and on-line video broadcast of operations using a mobile camera with a wireless broadcast module, which is fixed on the head.

Undoubtedly, the planning stage of surgical intervention is very important. The study and analysis of computed tomography using a package of specialized software has long been an accessible and uniquely informative method of additional examination. But sometimes it is not enough to perform calculations and evaluate the maxillofacial area in the form of sections in three mutually perpendicular planes. Material and technical capabilities of the department today with the help of a printer for rapid prototyping allow you to create stereolithographic models of different complexity, used in complex reconstructive interventions.

In May 2009 on the basis of the Central Military Hospital of the Ministry of Defense of Ukraine in Kyiv, where in the XIX century Professor Yu.K. Szymanowski (author of the monograph “Operations on the surface of the human body” (1865), 150 years since the publication of the department will celebrate in May 2015), and in the twentieth century. – S.S. Tigerstedt (who created a system of dental braces for the treatment of fractures of the jaws, which in 2015 will be 100 years old), will be held and the V Congress of craniofacial surgeons of Ukraine (with international participation) with the publication of printed materials.

Regarding international activities, in September 2008 the delegation of the department consisting of 4 people, headed by Professor V.O. Malanchuk, took part in the XIX Congress of the European Association of Craniofacial Surgeons (Bologna, Italy). Two young teachers and one clinical resident of the department met with many prominent experts from the world, listened to their reports, were introduced to the President of the Association, Professor L. Clauser (Italy) and recommended for membership in this Association.

In June 2009, Jennifer Strabel, a medical student at the University of Canada, worked for 2 weeks at the department, assisting teachers in operations, for which she wrote a letter of thanks on June 2009 to the rector of NMU, Professor V.F. Moskalenko.

Scientific and practical achievements have led to the fact that on August 21, 2000 the head of the department V.O. Malanchuk was awarded the Order of Merit of the III degree, in 2002 he was awarded the title of “Professor”, in 2003 he was elected a corresponding member of the National Academy of Medical Sciences of Ukraine, and in 2004 he was awarded the title of “Honored Worker of Science and Technology of Ukraine”.

The way of integration of the domestic specialty into the foreign space was continued by the delegates from Ukraine, Corresponding Member NAMS of Ukraine, prof. V.O. Malanchuk et al. N.M. Timoshchenko attended the XXI Congress of the EFSA, which took place on September 12-15, 2012 in Dubrovnik (Croatia). The congress was attended by about 1.5 thousand surgeons from 70 countries, including high-level specialists, who continue to develop the profession today, set the pace and direction: H. Obwegeser (Switzerland), H. Kawamoto, I. Jacson, E. Ellis (USA), H. Luhr (Germany), L. Clauser (Italy) and others. The main thematic approaches of the event: art in craniofacial surgery, beauty and psychology, benign lesions of facial bones, bisphosphonate diseases, reconstructive facial surgery, facial rejuvenation, microsurgical reconstruction of the head and neck, rhinoplasty, pre-prosthetic preparation and implantation, implantation, pathology fabric engineering.

In their lectures, the scientists covered the current trends in the treatment of combined injuries, the principles and sequence of medical care in the centers of the multidisciplinary trauma center, noted the need for in-depth examination of the victim, in-depth knowledge of anatomy and physiology, and not only technical and material support.

In the second part of the lecture the questions of dental implantation, interaction of artificial construction and biological tissues of an organism, biology of bone tissue, its changes in the conditions of loadings were raised.

Ongoing scientific and friendly cooperation with foreign colleagues and related specialists continues. Already on October 11-13, 2013, the head of the department, Professor VO Malanchuk and Assoc. O.L. Tsilenko took part in the first international conference on ortho-facial surgery and orthodontics, which took place in Moscow. In addition, 9 orthodontists from Ukraine were present. The rich scientific program of the forum and lively discussion once again confirmed the indisputability of joint work of maxillofacial surgeons and orthodontists in the treatment of patients with maxillofacial deformities and other pathologies of reconstructive surgery, the feasibility of their preoperative preparation and postoperative management. The issues discussed at the conference confirmed the relevance of the research direction of the department. In a relaxed atmosphere with a member of the cor. RAMS O.I. Nerobeev, prof. V.V. In December 2013, prof. Malanchuk VO was invited to give lectures to students of Vitebsk State Medical University (VSMU) on the topic: “Modern views on the problem of osteomyelitis of the jaws”, which highlights the scientific developments of the department in the study of osteomyelitis of the maxillofacial area, including its toxic form. Also, Belarusian colleagues expressed interest in the pedagogical experience of the dental faculty of NMU, some aspects of which were presented in the lecture “Development of professional competence of future doctors” (Assoc. Prof. Ya.A. Kulbashna).

On April 29, 2010 the Academic Council of the University recommended a set of scientific works of the staff of the department on biological substantiation of reconstructive-relative operations for participation in the competition for the Prize of the National Academy of Medical Sciences of Ukraine on regenerative maxillofacial surgery (V.O. Malanchuk, V.S. Astakhova, O.S. Volovar, I.P. Logvinenko, O.L. Tsilenko).

In 2004–2005, the department took the 3rd place according to the university rating, in 2007–2008 – the 5th place out of 48 among other clinical departments of NMU.

In total, during the work at the department were defended doctoral dissertations – 24, candidate – 97; published textbooks, monographs, collections of abstracts – 50; copyright certificates and patents over 215; scientific publications over 1450; conferences, congresses, congresses were organized, including with international participation – 19.

Given the multifaceted work of the department, the complexity of the specialty, significant scientific achievements and time requirements, in June 2005 the department submitted to the rector of the university a request to change the name of the department and possibly rename it “Department of Surgical Dentistry, Maxillofacial Plastic Surgery and implantation “.

This decision of the meeting of the staff of the department was supported by the methodical commission of the dental faculty and the university. In 2006–2007, the department worked under this name, but then it was replaced by the name that still exists today: Department of Oral and Maxillofacial Surgery.

Under the new conditions, the department continues to be the main (“supporting”) among the profile departments of the Ministry of Health of Ukraine. Textbooks have been published in English – prepared for printing), its specialists are responsible for the work of the medical aviation service in the profile, since 1985 we have been working 0.5 rates of maxillofacial surgeons in the ambulance hospital of Kyiv.

From January 21, 2013 to March 23, 2014 in the branch of KMKL № 12 at the initiative of the management were created specialized teams of medical care for victims in the center of the capital, provided round-the-clock duty of highly qualified specialists, including and employees of the department in conjunction with the work of volunteers who supplied food, medicine, essentials.

The enhanced regime was introduced on February 18-20, 2014. The work was carried out in conjunction with doctors of other specialties (vascular surgeons, traumatologists, ophthalmologists, neurosurgeons, neurologists, neurosurgeons, resuscitators, maxillofacial surgeons).

In total, from December 21, 2013 to March 23, 2014, 33 patients were admitted (self-treatment – 14, ambulance – 19), of which with inflammatory processes – 7 patients, gunshot wounds – 7, non-gunshot wounds – 14. Removed 12 artificial bodies from the deep parts of the face. 12 patients were hospitalized to the department, 21 patients received outpatient treatment. Also, emergency medical care was provided by the staff of the department in other locations (assistant Y.V Chepurny. together with other specialists operated on the victim, assistant N.A. Berezovska provided assistance to the wounded on Independence Square).

On December 3, 2014, Corresponding Member of the National Academy of Medical Sciences of Ukraine, Professor V.O. Malanchuk gave a lecture at the symposium “Peculiarities of surgical dental care in wartime and emergency medical care” as part of the lecture “Dentistry – 2014” of the Association of Dentists of Ukraine and the Ukrainian Association of Maxillofacial Surgeons, bringing to the attention of doctors and colleagues on the victims in the anti-terrorist operation zone, who were provided with dental care and focusing on some features of military operations and first aid of the so-called “hybrid war”.

The report was based on facts and clinical material concerning the victims of the Maidan confrontations and the wounded in the anti-terrorist operation zone. In the light of the last events operations to fighters of anti-terrorist operation are carried out and to this day…

Today the staff of the department has 2 professors, 8 associate professors, 23 assistants, 8 part-time assistants, postgraduate students – 4, clinical residents – 3, senior laboratory assistants – 7, laboratory assistants – 5. Active medical-consultative work, public and organizational activities are carried out, intensive pedagogical process.

On September 12-16, 2016, the delegation of the department took part in the XXIII Congress of the European Association of Cranio Maxillofacial Surgeons, attended by representatives from 78 countries. The scientific part of the event was presented by 430 oral reports and 938 posters.
