The Department of Social Medicine and Public Health
The important component in activity of departments is the scientific research activity. The directions of scientific researches at department are stipulated by specificity of its orientation, which anticipates teaching versatile issues on public health, organization of health care, biostatistics and economics of health care.
Scientific topics of department
The sphere of scientific interests at employees in department covers the problems on formation of policy and strategy for development of national health care system taking into consideration the world and European experience, extension of international relations in health care, improvement in organization of health care systems, their reforming, regulatory legal, personnel, financial and material technical provision, management and marketing in health care, strategic and special-purpose planning, management by activity in industry, complex intersectoral interbranch approach, medical care of population, health care of mothers and children, rural residents, contingents, who suffered from consequences of accident in Chernobyl NPP, etc., development of primary medical care due to principles of family medicine, improvement in activity of specialized medical services, medical diagnostic technologies in cardiology, provision with availability and quality of medical services, standardization, preservation and promotion of public health, improvement in determinant factors of health, researches on problems in ecological public health, struggle against social important and social dangerous diseases, formation of healthy lifestyle, consolidation of preventive direction in health care, liquidation of consequences from technological disasters in megapolis, development of medical law, bioethics and evidence medicine, regulatory and educational methodological provision, reforming higher medical education, implementation of modern new educational technologies within implementation of provisions from Bologna declaration.
Fulfillment of SRW
The scientific research work “Analysis on modern tendencies in development of health care in countries of European region” was performed during 2004-2008 at department. Within its limits they made the analysis and comparative assessment for demographic situation, state of health and lifestyle of population in Ukraine and other countries in WHO European region during 1985-2007.
The modern medical demographic tendencies were defined, including the state of premature mortality among population, laws on dynamics of prevalence and sickness rate of population, as well as social important diseases.
The issues on regulation of right on health care in international and European legislation, state of legal principles in health care systems in different countries were studied.
The assessment for peculiarities in organization, financing, resource provision and staff potential of health care systems, volumes in rendering medical aid to population in countries of WHO European region was performed.
This research allowed making the system scientific analysis of state, medical demographic tendencies in public health, reforms in health care systems in countries of WHO European region and grounding the strategic landmarks to improve the national health care system at modern stage.
Since 2009 the employees at department have been working on SRW “Scientific analysis on main determinants for public health”.
The laboratory of strategic researches in health care functions on the base of department, its employees perform the significant volume of scientific and publishing activity. The staff of laboratory includes 6 scientific workers, including academician of NAMS of Ukraine, Doctor of Medicine, professor Moskalenko V.F. (head), Doctor of Medicine, professor Gruzyeva T.S. (deputy head), leading scientific worker, Candidate of Medicine, assistant professor Galiyenko L.I., leading scientific worker Inshakova G.V., scientific worker Gruzyeva O.V. (photo).

The main scientific tasks of laboratory is the overall analysis on modern problems of public health, activity of world, European and national health care system, research of their different models, grounding the strategy and tactics for development of industry, its legal, resources’ provision, development of draft concepts, programs, regulatory legal acts, formation of preventive principles in health care.
Every year the laboratory publishes the handbook “Public health in Ukraine”, other analytical, informative materials, monographs and scientific articles in Ukraine and abroad.
The head at department, academician of NAMS of Ukraine, Doctor of Medicine, professor Moskalenko V.F. took and takes part in fulfillment of many scientific research works, carries outthe scientific supervision for 8 works, including order from MHC of Ukraine on scientific accompaniment of Interbranch complex program “Health of nation” for 2002-2011.

Professor at department, Doctor of Medicine, professor Gruzyeva T.S. is the responsible executive of SRW:

The employees of department and laboratory on strategic researches in health care closely cooperate in salutation of scientific problems with many international agencies, including WHO Regional office in Europe, International fund “Revival”, Centers for Disease Control and Prevention (CDC, USA).
Thus, the employees of department made the following scientific works due to the order from ROE and CDC:
- “Elaboration and editing National report “Tobacco control in Ukraine”;
- “Global survey of medical students about tobacco smoking”;
- “Development of educational programs on problems of tobacco smoking”.
At the order from WHO ROE they implemented the scientific project “Review of national data on peculiarities in nutrition of different groups of population in Ukraine and extension”. Within cooperation with International fund “Revival” they perform the scientific project “Grounding, development and pilot implementation of academic program for academic discipline “Medical law” at pre-graduation level in higher medical schools in Ukraine”.
Results from fulfillment of SRW
The new approaches to reforming medical sphere were developed and implemented due to results from scientific developments, made by employees of department and laboratory on strategic researches in health care under supervision of academician Moskalenko V.F., as well as the special-purpose method was launched and actively implemented into health care. The Concept on development of health care among population of Ukraine, approved by Decree from President of Ukraine in 2000, Complex measures for implementation of family medicine into health care system and Interbranch complex program “Health of nation” for 2002-2011, approved by decrees from Cabinet of Ministers of Ukraine in 2000 and 2002, conceptual model for establishment of regulatory legal base of health care in Ukraine was developed under direct participation and supervision of V.F. Moskalenko.
The scientific developments underlay the Concept on formation of uniform medical space, Concept on development of family medicine in Ukraine, Concept on development of health care among population in c. Kyiv, Concept on development of higher medical education in Ukraine, Concept on development of quality management system for medical aid to population of Ukraine, rating system on assessment of health care control bodies at regional level due to indices for state of public health, activity and resources’ provision of health care establishments, as well as rating system on assessment for activity of higher medical schools.
The principles and approaches to establishment of optimal model for health care system, was grounded, as well as the algorithm for its construction was suggested, the conditions for success in its realization were defined within scientific accompaniment of Interbranch complex program “Health of nation” for 2002-2011 in the laboratory on strategic researches at Bogomolets NMU under supervision of academician of NAMS of Ukraine, professor Moskalenko V.F.
The principles for construction of optimal model for health care system were explained in reports at the meeting of Academic medical council of MHC of Ukraine (12.12.2007), IV congress of specialists in social medicine and organizers of health care (22.10.2008), open lecture in NMU (28.12.2009), in monograph by Moskalenko V.F. “Principles for construction of optimal model for health care system: European measurement”, in publications in newspapers “Vashe zdorovya”, “Meditsynskaya gazeta” (RF).
The scientific researches proved the necessity in intensification of preventive direction in health care, transition from medical prevention to social prevention, scientifically grounded the conceptual principles for modern preventive strategy onglobal complex integral intersectoral interbranch preventive system of vertical and horizontal direction and the ones that are set in monograph by Moskalenko V.F. “Conceptual principles for formation of preventive strategies in health care”
The possibilities for prevalence of tobacco smoking among students at higher medical schools within joint international project under the aegis of WHO and US Center for Disease Control and Prevention were studied. For the first time in Ukraine they grounded and developed the educational course for students in higher medical schools “Tobacco control”, which covers the toxicological, clinical, medical social, legal, economic issues and issues on prevention and rendering aid in refusal from smoking. The topical lecture course on problems of prevention from tobacco smoking. The scientific developments on this issue were used while preparing the National report “Tobacco control in Ukraine”, which are presented in WHO ROE.
2 round tables with participation of representatives from MES of Ukraine, MHC of Ukraine, leading scientists and students were held together with specialists from WHO ROE, WHO Bureau in Ukraine and Center for Disease Control and Prevention (USA) (21.10.2009, 28.09.2010).
The influence of character and quality of nutrition on public sickness rate of chronic non-infectious diseases was determined; the corresponding analytical report was prepared and presented to WHO ROE.
The academic program for academic discipline “Medical law, which was approved by CMC HE MHC of Ukraine, was grounded and developed according to analysis of international and national experience, recommendations of international and regional organizations.
The medical, sanitary hygienic and epidemiological aspects of emergency in the megapolis as a result of breakdowns were defined pursuant to researches on modern genesis and peculiarities for technological and natural catastrophes,four-factor concept for liquidation of their medical social consequences were scientifically grounded. The main principles were defined and the model for system approach to assessment of ecological catastrophes, formation and realization of liquidation measures for the case of technological breakdowns was developed, the new classification for emergencies of natural and technological character was offered for use.
The concept on system of continuous professional development due to specialty “general practice-family medicine” and its scientific methodological basics, special academic program on continuous professional development of doctors due to specialty “general practice – family medicine”, standards and structural logical scheme for its realization was developed within research of issues on scientific methodological accompaniment of training system for specialists in family medicine.
A number of regulatory, methodological and informative documents to provide with implementation of credit module system on organization of academic activity within the context of requirements in Bologna declaration was grounded and established within the direction of reforminghigher medical education in Ukraine.
Pursuant to study and generalization of world, European and national experience, Moskalenko V.F. grounded the optimal model for health care system and algorithm for its construction that is confirmed by copyright for work.
The principles for construction of optimal model for health care system are described in reports in IV congress of specialists in social medicine and organizers of health care.
The results of scientific developments became the grounds for study and establishment of

- Methodological approaches to formation of concept on reduction of premature mortality;
- Conceptual approaches to formation of integral system on global vertical and horizontal prevention;
- System on organization of preventive activity in establishments of general practice / family medicine; conceptual and functional model for preventive activity of family doctor;
- Model for preventive basics to avoid the development of inborn defects;
- Complex measures on health care of children and provision with healthy environment;
- Monitoring system on health care of population and environmental protection;
– methodological approaches to formation of program on long term observation for influence of air pollution on formation of allergic diseases at children;
- Functional model to forecast risk factors for possible losses of children, aged less 1 year;
- Functional model for preservation and improvement of inborn’ health;
- Directions in re-structurization of city ambulatory dental service, principles and mechanisms for improvement of primary dental aid to urban population due to principles of family medicine, management, quality management system;
- Conceptual model for re-structiruzation and management by system of ambulatory dental aid to urban population;
- Ways to assess the efficiency in treatment of generalized periodontitis and dispensary monitoring for patients, way to assess the affection of paradontium at patients with generalized periodontitis;
- Formation of academic programs for academic disciplines “medical law” and “management in health care” to be taught in higher medical schools;
- Principles for organization of independent problem-based work of students on urgent issues of social medicine.
Due to results from scientific researches more than 1.5 ths. scientific works were published, including over 40 monographs, a great number of guidelines and instructions, made over 200 reports and speeches at scientific forums at different levels, in 24 countries abroad, in particular in Great Britain, Georgia, Spain, Macedonia, RSA, Poland, Portugal, Russian Federation, USA, Croatia, France and Sweden.
Organization and performance of scientific forums
Every year the employees of department took an active part in organization and performance of scientific practical events on occasion of World Health Day.
In 2005 – it was the University’ action on diagnostics of level of individual health and rendering recommendations for preservation and promotions of health.
In 2006 they held the seminar in commemoration of World Health Day under slogan “Work for the benefit of health – our common task”, which was devoted to the problem on staff provision of health care. The scientists of NMU took part in the seminar together with the scientists from National university “Kyiv-Mohyla Academy”, National academy of state management at President of Ukraine, Ukrainian institute of public health, P.L. Shupyka National medical academy of post-graduation education.
The first, really scientific marathon, devoted to World Health Day, testified: Kyiv, within the scientific potential of National medical university, has the significant grounds to hold such scientific forums, running beyond national limits.
In autumn 2006 the university together with WHO Regional office in Europe held the seminar on issues for use of tolls and methods to assess staff resources and to form staff policy in health care in the countries. The representatives from 5 continents took part in the seminar, including Europe, Asia, Australia, Northern and Southern America, 29 countries in the world, including 25 countries in European region of WHO, as well as Australia, Brazil and Canada.
The seminar in the history of NMU became a regular important stage in improvement of staff training system for health care, interesting strategic proposals were implemented.
In 2007 the International conference on occasion of World Health Day 2007 was held in Ukraine together with WHO ROE and WHO Bureau in Ukraine. It was devoted to international safety in the sphere of health care and was held under slogan “Invest in health, build a safer future”.
Over 200 persons from 6 countries took part in the conference, including Denmark, China, Poland, Russian Federation, USA, Ukraine and three continents – Europe, Asia, America. The welcome speeches were made by first deputy minister of health care of Ukraine, Councilor of WHO ROE, professor at Alabama University in Birmingham (USA), head of WHO Bureau in Ukraine and rector of Bogomolets NMU.
Among 27 reports in plenary meeting of the conference the prominent one was made by councilor in European bureau of WHO Jerald Rokenshaub. NMU presented 9 reports, including “Priority directions in contraction to global threats to public health”, which was made by academician of NAMS of Ukraine, professor V.F. Moskalenko.
The reports by specialists from other scientific and educational establishments, in particular, scientists of NAS and AMS of Ukraine were of great scientific interest. As well 17 poster reports were presented, including from Scientific research institute of public health at Russian academy of medical sciences.
The beginning of April 2008 was marked by important event – performance of International conference, devoted to 60th anniversary of WHO, World Health Day and health protection against changes in climate. The co-organizers of conference were Ministry of health care of Ukraine, Bogomolets National medical university, WHO Bureau in Ukraine. The conference was preceded by intensive preparatory work on formation of its program, publication of materials of reports, worries about accompaniment of responsible international action. But it is the fixed style in NMU. On 7, April 2008 the university warmly welcomed the scientists from many cities and countries, adherents and followers to idea on strategy for achievement of health for all.
Over 300 participants from 12 countries in the works came to share their scientific works, including from Belarus, Denmark, Iran, Kazakhstan, Latvia, Lithuania, Germany, Poland, Russian Federation, USA, Ukraine, Sweden. The Minister of health care solemnly opened the conference. The sincere welcome words from AMS of Ukraine were expressed by Leonid Georgiyovych Rozenfeld, from Ministry of environmental protection – Deputy Minister Mykola Mykhaylovych Movchan.
The participants in the conference were passionately welcomed by foreign colleagues, councilor of WHO Regional office in Europe Agis Zuros, deputy director on scientific activity at SRI of public health and management of health care, head at department of public care and health care with course of economics at I.M. Sechenov Moscow medical academy, corresponding member of RAMS, professor Volodymyr Zakharovych Kucherenko, professor at department of public health and health care at Saint Petersburg state medical university Nataliia Guriyivna Petrova.
35 reports by leading specialists from different countries in the world were made at plenary meetings. Their topics covered a wide range of issues, including protection of citizens against changes in climate, ecological problems of public health, consolidation of health care systems, modern strategies for development, peculiarities and tendencies in population health, new challenges and threats, struggle against infectious diseases, intensification of epidemiological supervision, demographic problems of health, population growing old, factors that influence on formation of health, preventive strategies in health care, availability and quality of medical aid, primary medical care, family medicine, social important and social dangerous diseases, legal and ethical principles of health care, management and financing of health care.
The participants listened to the program reports by councilor of WHO ROE Agis Zuros on the topic “Problem of non-infectious diseases in European region” and head of WHO Bureau in Ukraine Pokanevych on the topic “WHO in the world and European region” with the great interest.

The special attention was paid to the report by leading specialists of Ukraine in the sphere of medical ecology, academician of AMS of Ukraine Serdiuk A.M. and Turos O.I. “Changes in climate: monitoring, risks, forecast”. The main principles for management of clinical practice and clinical management were described in very comprehensive report “Management of clinical practice: clinical management” by corresponding member of RAMS, professor Kucherenko V.Z.

Provide with safety of hospital in emergencies”, was held at NMU with participation of representatives from Ministry of health care of Ukraine, executive at Ministry of Ukraine on issues of emergencies and on affairs of population protection against consequences from Chernobyl catastrophe, Chairman of National council on issues of public health care at President of Ukraine, regional councilor of WHO ROE on staff issues (Denmark), professors of Alabama University in Birmingham (USA), Caroline institute (Sweden), head in WHO Bureau in Ukraine, chief sanitary doctor of State sanitary department of Hungary, director in Ukrainian scientific practical; center of emergency medical aid and emergency medicine at MHC of Ukraine, representative of scientific and educational establishments from 14 countries of the world (07-08.04.2009).

The main thematic event, devoted to World Health Day 2010, was the International scientific practical conference under slogan “1000 cities 1000 lives. Health of cities is important”. Bogomolets NMU gathered the leading specialists in this sphere from 14 countries in the world, including Afghanistan, Azerbaijan, Belarus, Malaysia, Kazakhstan, China, Iran, Lithuania, Moldova, Poland, Russian Federation, Ukraine, Switzerland, Sweden to discuss a wide range of problems on urbanization and health within International scientific practical conference, devoted to World Health Day 2010.
The conference was opened by rector of Bogomolets NMU, academician of NAMS of Ukraine, professor Moskalenko V.F. The welcome speeches to participants in forum were made by the First Deputy Minister of health care of Ukraine, Chief state sanitary doctor of Ukraine, head in WHO Bureau in Ukraine Pokanevych I.V., Vice-president of AMS of Ukraine, academician Zozulya Yu.P. The participation of representatives from priesthood was of great educational importance. 38 reports were presented at plenary meetings. The representative of WHO ROE, head in WHO Bureau in Ukraine Pokanevych I.V. made the program report “Urbanization and health”. Famous Swedish scientist, professor in Caroline institute Y. Pershagen made the deep and comprehensive report “Ecological problems, being important for public health – how to establish the priorities in the sphere of prevention?” within the context of topics for World Health Day 2010. The issues on ecological public health were also raised in reports by professor in O.M. Marzeyev Institute of hygiene and medical ecology at AMS of Ukraine Turos O.I. and chief doctor of Central SES of MHC of Ukraine Nekrasova L.S.
The report by director of Regional public organization “Association of medical societies on quality” Ulumbekova G.E. on topic “Lessons from reforming health care in Russia” was listened with great attention and significant interest. The head at department of social medicine, organization and management of health care in Dnipropetrovsk state medical academy, professor Lekhan V.M. in her report described the deep critical analysis on functioning health care system in Ukraine. The director in Ukrainian institute of strategic researches at MHC of Ukraine, professor Slabkyy G.O. outlined the modern directions in reforming health care system in Ukraine.
The report by rector in P.L. Shupyka National medical academy of post-graduate education, professor Voronenko Yu.V. on topic “New approaches to training specialists due to specialty “organization and management of health care” was of extreme interest for scientists and workers in practical health care. The scientific notification by head at department of public health care in National academy of state management at President of Ukraine, professor Solonenko and coauthors presented the role of local government authorities in improvement of efficiency in public health care in Ukraine.
The powerful medical legal direction of the Conference was opened by fundamental report from corresponding member of AMS of Ukraine, professor Moskalenko V.F. and coauthors on topic “Urgent issues in teaching medical law at medical law higher schools”. It described the modern approaches, used to ground and to develop the academic program for academic discipline “medical law” for higher medical schools in Ukraine, which was approved by MHC of Ukraine in 2009. The deep analysis on provisions in ethical code of doctor in Ukraine was made in report by assistant professor in Lviv Danylo Galytskyy national medical university, president in all-Ukrainian public organization “Foundation of medical law and bioethics of Ukraine”, Candidate of Law Seniuta I.Ya. The regulation of criminal liability for medical crimes was disclosed in report by assistant professor in Academy of MIA of Republic of Moldova, Candidate of Law Flori V.M. The report by head at legal department in all-Ukrainian Council on protection of rights and safety of patients Skoryna O.O.
Traditionally, the Conference, devoted to World Health Day, presented the scientific developments, which disclose the modern tendencies in peculiarities for public health in interconnection with the lifestyle. They concerned rational nutrition, struggle against bad habits, including tobacco smoking, peculiarities for formation of health at certain groups of population, influence of stress factors under conditions of urbanization, etc.
The significant attention was paid to issues on organization of medical aid, implementation of modern organizational medical technologies into health care, including primary medical care, family medicine, its resources’ provision, actual problems on development of palliative and hospice aid.

The program of Conference also included 16 poster reports on different aspects from influence of urbanization on health and public health care. The reports by leading specialists in A.D. Sakharov International state ecological university (Belarus) were of significant interest. The experience from research of epidemiological peculiarities for poisoning in Republic of Azerbaijan and development of scientific principles for their prevention was presented in report by scientists from Clinical medical center in c. Baku (Azerbaijan).
The Conference was closed in solemn atmosphere, at which Minister of health care of Ukraine Zinoviy Mykolayovych Mytnyk made the speech and presented the diploma for the best scientific works and diploma of winners in II stage of all-Ukrainian students’ Olympiad on academic discipline “social medicine and organization of health care”.

The department of social medicine and health care celebrated its 85th anniversary in January 2009. The International scientific practical conference “Health and health care in XXI century: problems and ways for solution” was devoted to that event.
