The priority areas of scientific work of the Department of Internal Medicine №4 are the following sections of modern cardiology, rheumatology and internal medicine:
- Diagnosis and treatment of patients with comorbid pathology of internal organs
- Imbalance of amino acid composition of blood plasma as a possible pathogenetic link in the development of atherosclerosis and ischemic heart disease
- Treatment of cardiac arrhythmias in post-infarction patients with type 2 diabetes
- Features of treatment of patients with generalized atherosclerosis
- State of the composition of intestinal microbiota in patients with ischemic heart disease with paroxysmal atrial fibrillation
- Role of impaired tryptophan and serotonin metabolism in patients with generalized atherosclerosis
- Role of impaired humoral and cellular immunity in the development of atherosclerosis and acute coronary syndrome
- Prediction of the course of unstable angina by studying the fatty acid spectrum of platelets
- Features of diagnosis and treatment of ischemic heart disease in patients with concomitant pancreatitis
- Role of impaired fatty acid spectrum of lymphocytes in the development of ischemic heart disease
- State activity of the dopaminergic system as one of the factors in the development of arterial hypertension and obesity

The topic of the research project of the Department of Internal Medicine No. 4 is “Features of lipid, protein, neurotransmitter metabolism and hemodynamics in patients with generalized atherosclerosis and secondary prevention of cardiovascular events”. All scientific and pedagogical workers of the department take an active part in the research project. In 2024-2025 academic year, the scientific and pedagogical workers of the Department of Internal Medicine No. 4 published: 24 articles (15 of which were published in the scientometric databases Scopus and Web of Science), 23 abstracts (19 of which are international). The results of the research project were covered in reports at Ukrainian and international conferences. Professors and associate professors of the department are members of specialized councils for the defense of dissertations; 2 doctoral dissertations and 5 dissertations for the Ph.D. degree are being carried out at the department.
The research work of the Department Internal Medicine No. 4 is devoted to the study of the mechanisms of development and progression of generalized atherosclerosis – a complex multifactorial process, which is the leading cause of cardiovascular events, such as heart attacks and strokes.
The aim of the study was to identify key biological risk factors through a comprehensive analysis of the level of lipids, proteins, changes in neurotransmitters and hemodynamics in patients with generalized atherosclerosis and to improve methods of secondary prevention of cardiovascular events for patients with multifocal atherosclerosis
Within the reporting stage, patients with various forms of ischemic heart disease and associated chronic diseases were examined. Indicators of lipid and protein metabolism, the activity of inflammatory processes, the state of the endothelium and vascular wall, as well as the levels of neurotransmitters (serotonin, histamine, dopamine) and matrix metalloproteinases (MMP) – enzymes that affect the structure of blood vessels were determined.
The works of the authors of the Department of Internal Medicine No. 4 showed that the imbalance of neurotransmitters is closely related to increased MMP activity and the rate of vascular remodeling, which directly affects the risk of repeated cardiovascular events. Based on these data, an integrated approach to secondary prevention is being developed, which includes: correction of lipid and protein metabolism; regulation of inflammatory processes; normalization of neurotransmitter balance; improvement of hemodynamics and vascular function.
The implementation of a comprehensive strategy allowed to significantly reduce inflammatory markers, stabilize the structure of the vascular wall and improve blood supply to the heart and brain.
The results emphasize that secondary prevention of cardiovascular events should be multicomponent and personalized. They are important for the health care system, social stability and the economy of the country, as they contribute to reducing complications and increasing the working capacity of the population.
